If you are interested in our services, please fill out this form or call 619-594-5134. You will be asked to leave a message with your name, phone number and a brief description of the services you are interested in receiving. Please also include information about the best time to reach you for a return phone call.
Note: Due to overwhelming demand, our assessment clinic is not able to accept new patients at this time. Our waitlist for assessments for ADHD and learning disabilities (for both adults and children) is currently closed. The interest form will be updated when the assessment waitlist reopens.
After the interest form is received, the clinic will contact you to complete a screening call (which will take approximately :30 minutes) in which a clinician will ask you follow up questions, learn more about the services you are interested in, and determine whether the clinic is able to serve your needs.

This clinic is not affiliated with SDSU Counseling & Psychological Services that is provided for students on campus. We are fee-based and serve the community.
